IMWG consensus criteria for response and minimal residual disease in myeloma

This paper, produced by an expert panel of the International Myeloma Working Group (IMWG) reviews the assessment of disease response and endpoints for clinical trials, and provides updated response categories of minimal residual disease negativity.

Over the past decade, the treatment landscape for multiple myeloma has evolved significantly, largely due to the introduction of various new and effective drug classes that have markedly improved both response rates and the depth of response in patients.

Traditionally, response criteria in multiple myeloma relied on serum and urine assessments of monoclonal proteins, as well as bone marrow evaluations, which have been recognised as relatively insensitive. With the advent of innovative treatment strategies leading to higher rates of complete response, there is a pressing need to establish new response categories capable of identifying deeper responses than those conventionally classified as complete response.

Recent efforts have concentrated on detecting residual tumour cells within the bone marrow through advanced techniques such as flow cytometry and gene sequencing. Additionally, sensitive imaging modalities can help identify the presence of residual disease outside the bone marrow. By integrating these novel methods, the IMWG has introduced new response categories for minimal residual disease negativity, which can be evaluated alongside imaging-based assessments for the absence of extramedullary disease. This approach aims to facilitate uniform reporting standards both within clinical trials and in broader clinical practice.
