IMWG consensus guidelines and recommendations for the management and response assessment of chimeric antigen receptor T-cell therapy in clinical practice for relapsed and refractory multiple myeloma

CAR T-cell therapy represents an innovative approach for patients with advanced, treatment-resistant multiple myeloma, showing promising response rates between 73% and 98%. Currently, three CAR T-cell products have received approval in different regions: Idecabtagene vicleucel (ide-cel) and ciltacabtagene autoleucel (cilta-cel) are approved in the United States, Europe, Canada (ide-cel only), and Brazil (cilta-cel only), while equecabtagene autoleucel (eque-cel) has been approved in China.

CAR T-cell therapy differs significantly from traditional myeloma treatments, with unique side effects that demand specialised management strategies. To address these, the International Myeloma Working Group convened a panel of experts in haematopoietic cell transplantation, cell therapy, and myeloma care to establish guidelines for clinical use. Their consensus opinion provides essential insights to support healthcare professionals in safely and effectively implementing CAR T-cell therapy for myeloma patients.
