Last year was an unprecedented year for myeloma with four new treatments approved by the National Institute of Health and Care Excellence (NICE) and the Scottish Medicines Consortium (SMC) across the UK and the successful isatuximab appeal. In 2025, we believe there will be more good news for the myeloma community with five on-going appraisals at NICE.

We are also excited to see two National Cancer Medicines Advisory Group (NCMAG) reviews in Scotland which could see a huge step towards a more flexible, patient-centred pathway in Scotland.

Recent approvals: Recommended by NICE

Teclistamab (Tecvayli®) monotherapy recommended for relapsed and refractory myeloma patients who have had three or more lines of treatment including an immunomodulatory drug, a proteosome inhibitor and an anti-CD38 antibody. Read more about NICE lifting restrictions on teclistamab here.

Elranatamab (Elrexfio®) monotherapy recommended for relapsed and refractory myeloma patients who have had three or more lines of treatment including an immunomodulatory drug, a proteosome inhibitor and an anti-CD38 antibody. Read more about NICE lifting restrictions on elranatamab here.

Recent approvals: Recommended by SMC

Selinexor (Nexpovio®), bortezomib and dexamethasone (SVD) approved for relapsed and refractory myeloma patients who have had at least one line of treatment, are refractory to lenalidomide (Revlimid®) when an anti-CD38 monoclonal antibody is not appropriate.

Selinexor and dexamethasone (SD) approved for relapsed and refractory myeloma patients who have received at least four treatments including two proteasome inhibitors, two immunomodulatory drugs and an anti-CD38 monoclonal antibody.

Forward look: NICE

Isatuximab (Sarclisa®), pomalidomide (Imnovid®) and dexamethasone

In June, NICE decided not to recommend isatuximab with pomalidomide and dexamethasone for continued use on the NHS in England and Wales following a reappraisal. Myeloma UK and the UK Myeloma Society successfully appealed the decision.

The appraisal of IsaPD has now been referred back to the committee and a third committee meeting will be held in the coming months.

Belantamab mafodotin (Blenrep®), bortezomib (Velcade®) and dexamethasone

NICE is appraising belantamab mafodotin, bortezomib and dexamethasone for relapsed or refractory myeloma patients who have had one or more previous treatments. Myeloma UK is involved in this appraisal and submitted evidence on behalf of patients in May 2024. The NICE committee meeting took place on 8 January 2025. We await NICE’s decision.

Belantamab mafodotin, pomalidomde and dexamethasone

NICE has started the appraisal of belantamab mafodotin, pomalidomide and dexamethasone for relapsed or refractory myeloma patients who have had one or more previous treatments. Myeloma UK will submit evidence on behalf of patients in August 2024. The NICE committee meeting took place in January 2025. We await NICE’s decision.

Isatuximab, bortezomib, lenalidomide and dexamethasone.

NICE has started the appraisal for isatuximab with bortezomib, lenalidomide and dexamethasone for newly diagnosed myeloma patients who are not eligible for stem cell transplantation. Myeloma UK submitted evidence for this appraisal in October 2024 and will attend the first committee meeting on 2 April 2025.

Talquetamab (Talvey®) monotherapy

NICE has started the appraisal for the use of talquetamab monotherapy for relapsed and refractory myeloma patients who have had three or more lines of treatment including an immunomodulatory drug, a proteosome inhibitor and an anti-CD38 antibody. The deadline for evidence submission is 17 March and the first committee meeting will be held on 12 August.

Forward look: NCMAG

Pomalidomide, bortezomib and dexamethasone

The NCMAG is reviewing the off-patent use of pomalidomide, bortezomib and dexamethasone for myeloma patients who have received at least one prior treatment including lenalidomide. The council meeting assessing the application took place in December 2024 and their decision should be published at the end of January 2025.

Pomalidomide and dexamethasone

The NCMAG is reviewing the off-patent and off-label use of pomalidomide and dexamethasone for myeloma patients who have received at least one prior treatment including lenalidomide. This indication was one of the interim treatments approved during the COIVD-19 pandemic. The council meeting assessing the application took place in December 2024 and their decision should be published at the end of January 2025.
